In most instances, your doctor can complete your medical assessment online, saving you a trip to a Service NSW Centre to hand in a paper form. If you do have an eye condition it is a requirement of the RMS that you have your eyes checked by the Optometrist/Ophthalmologist. Please arrange this before booking an appointment with your GP. Forward the paperwork from the Optometrist/Ophthalmologist to reception@gpcremorne.com.au or hand to the receptionist BEFORE your appointment. This will minimise delays in submission of your RMS form.
