The Australian Type 2 Diabetes Risk Assessment Tool was developed to help reduce the risk of type 2 diabetes.
To assess your risk please download the tool or ask one of our staff for a form at the practice.
Complete the form and bring it to your GP to discuss your risk level.
What is Type 2 diabetes?
- It occurs when the body doesn’t produce enough insulin (a hormone produced by the pancreas) or respond well enough to insulin.
- Type 2 diabetes is a chronic disease marked by high levels of sugar in your blood.
- You can have type 2 diabetes and not know it because there may be no obvious symptoms.
Risk factors
- Over 40 years of age.
- Lifestyle factors: physical inactivity & poor nutrition.
- Family history
